Operating Standard:
Parental consent is necessary in order to record the name, likeness, voice, participation, comments and/or appearance of any minor in video, sound, photographic or written material.
The Acknowledgment of Risk and Waiver of Liability Form authorizes the university to record, exhibit and distribute a child’s participation and appearance for any educational or promotional purpose that is tied to a specific activity.
For any recorded media that may be shared outside of OSU, units should secure explicit written consent from a parent or legal guardian using the Model and Information Release Form.
Parents may opt to prevent their child's participation and appearance from being used in any recorded medium by completing the Photo Opt Out Release Form, which should be presented to all parents at the time of registration or application.
Evidence or Documentation:
- Copy of blank registration form or application, with option to submit a Photo Opt Out form.
- Randomly selected completed waivers.

- Any OSU unit or program that collects data, media, or biographical information of minors.
