In general, written consent should be sought from a parent or legal guardian for any unaccompanied minor (under age 18) or groups of minors participating in a program or activity operated by OSU or its partners. Colleges and departmental units should work with their internal compliance staff or administrative office to determine the appropriate type of liability waiver or consent, based on conditions of the environment and the scope of risks involved in the activity. The Acknowledgement of Risk (AOR) form is used to inform participants about potential risks involved with an activity, and to document their acceptance of those risks. In the case of youth programs, the AOR form must be signed by a parent or legal guardian for every minor participant. For youth volunteers, a parent or legal guardian must give consent prior to their child's involvement in sanctioned volunteer activities.
