In general, written consent should be sought from a parent or legal guardian for any unaccompanied minor (under age 18) or groups of minors participating in a program or activity operated by OSU or its partners. Colleges and departmental units should work with their internal compliance staff or administrative office to determine the appropriate type of liability waiver or consent, based on conditions of the environment and the scope of risks involved in the activity. For consultation or assistance with developing new waivers and consent forms, contact Enterprise Risk Services at firstname.lastname@example.org or call (541) 737-7252.
The Acknowledgement of Risk (AOR) form is used to inform participants about potential risks involved with an activity, and to document their acceptance of those risks. In the case of youth programs, the AOR form must be signed by a parent or legal guardian for every minor participant. For more information about AOR and Waiver of Liability, visit the Risk Services Waivers page.
Acknowledgement of Risk and Waiver of Liability (GROUP) to include the Property Owner (for activities hosted off university property)
All volunteers must complete the Conditions of Volunteer Service Form to be considered a registered university volunteer. For youth volunteers, a parent or legal guardian must give consent prior to their child's involvement in sanctioned volunteer activities. For questions about criteria and risk assessment related to volunteer activities, visit the Risk Services Volunteer page.

Consent is necessary in order to record the name, likeness, voice, participation, comments and/or appearance of any individual in video, sound, photographic or written material. In the case of minor participants, a parent or legal guardian must authorize this release. Parents may also opt to prevent their child's participation or appearance from being used in any recorded medium by using the Photo Opt Out form.
